Transitional Care Health Coach

Health coaches are registered nurses located in selected clinics, hospitals and/or home based that provide education, support and self-management tools aimed at improving the member’s health.

Typical Hours: 8-4:30pm, M-F

Health Coaching is one of the fastest-growing industries in the healthcare field. In the role of a Health Coach, you will have the opportunity to positively impact your clients health and wellbeing.

The Transitional Care Health Coach is responsible for providing support, education, and guidance to patients as they transition from hospital to home or between different care settings. This role aims to improve patient outcomes, reduce hospital readmissions, and enhance overall quality of care.
